The Brain is Pathogenic’s hidden final area. You can’t enter it on a brand-new save. First, you must unlock the game’s first ending, then solve several hidden puzzles across multiple stages during a single run.
The steps below combine the original discovery with information confirmed by the community. A few mechanics are still partly RNG, but this method has worked for many players.
Note: Some details still aren’t fully confirmed. The developers haven’t explained every requirement, and a few events appear to depend on RNG.
How To Access The Brain For Secret Ending in Pathogenic
Step 1 – Unlock the First Ending
Before anything else, defeat the Heart boss and finish the game once.
The ending cutscene shows a sequence of arrow inputs. You must remember this code because you’ll use it later.
The default sequence is:

Step 2 – Find Every Sigil Room
You need to locate hidden sigils from Skin through Lungs.
Each location has one sigil puzzle.
Skin
The Skin sigil sits inside a secret room.
Many players initially searched every wall, but the developer later confirmed that you don’t have to.
Instead, watch for walls with the same decorative bumps used on hallway entrances. Those decorations only appear the first time you enter a room, so pay attention before you start shooting.
If you still can’t spot them, firing at suspicious walls also works. Fast weapons make this much easier, and melee attacks produce a different sound when they hit a secret wall.
Intestines
Defeat the Intestines boss.
A hidden vein can appear afterward.
Community testing shows this spawn is RNG for many players. Some runs simply won’t create the vein.
However, several players reported that carrying the Antenna/Sensor organelle guarantees the vein appears after the boss.
Stomach
The Stomach sigils stay completely hidden.
You need the special organelle from an earlier sigil room before you can locate them.
Some players also reported that Cytokine Storm reveals the hidden entrance when the sigils spawn inside the storm.
Liver
The sigils appear inside the Collector’s Room.
You can kill the Collector without breaking the puzzle.

Step 4 – Get the Special Organelle
The first completed sigil puzzle rewards a unique organelle.
Early reports claimed it always appeared in Skin, but newer testing suggests it actually spawns in the first sigil room you complete, regardless of the stage.
Players often call this item the Sensor or Antenna because its real name isn’t obvious.
Before it works, charge it at a Mitochondria.
Once charged, it shines a blue-white beam toward the next hidden sigil location. This makes the remaining puzzles much easier to find.
Step 5 – Collect the Four Yellow Shapes
Each hidden sigil room contains a yellow shape.
Pick up all four during the same run.
You must collect every shape before defeating the Heart again.
Step 6 – Return to the Heart
After collecting every yellow shape, defeat the Heart boss in the same run.
Walk to the top of the Heart arena.
A row of gray placeholders should appear.
Step 7 – Insert the Yellow Shapes
Stand in front of the gray symbols.
Use the Arrow Keys to place the yellow shapes.

The yellow shape tied to Up, Down, Left, or Right is random.
