There are 4 endings you can unlock in Sister Other Paranoia, along with 25 CGs for the image gallery.
The game has five routes that you can select from the panopticon screen. You don’t need to complete them in a specific order, and you can switch between routes while completing their segments.
However, there are a few requirements for the alternate endings, so follow this guide if you want to unlock everything.
How the Routes Work
Each of the five routes has 4 segments.
You can complete the routes in any order. You also don’t have to finish one route before starting another.
For example, you can select the Bat route, then choose a different route on your next segment. The game tracks your completed segments rather than forcing you to finish one route at a time.
There is one important exception:
The final stage of the Ring route will only be available when you have completed all the other routes.
Sister Other Paranoia – Normal Ending
Complete the five routes to reach the Normal Ending.
You can work on the routes in any order, but you need to complete all of their required segments.
CGs Unlocked
- 1 – Complete the prologue.
- 7, 8, 9 – Complete the Fox route.
- 10, 11, 12 – Complete the Monkey route.
- 14, 15 – Complete the Caterpillar route.
- 16, 17, 18 – Complete the Bat route.
- 19, 20 – Complete the Ring route.
- 2 – Complete any route.
- 6 – Get the Normal Ending.
Monkey Route Choice
You may notice that the “He’s not a bad guy” choice is unavailable during your first playthrough.
This choice only appears when you’ve completed the Normal Ending.
So don’t worry if you cannot select it during your first run.
Caterpillar Route Choice
During the Caterpillar route, choose:
“You can do it. You can fly.”
This is required to complete the route and reach the Normal Ending.
True Ending
After getting the Normal Ending, return to the title screen.
Click on Sahana to start the True Ending sequence.
If you click on Sahana before getting the Normal Ending, you will only see a short cutscene of petting her head.
CGs Unlocked
- 3, 4 – Progress through the True Ending normally.
- 5 – In the sunflower field, wait when you get the option to look into her mind.
After waiting for a while, a new choice will appear:
“Trust my little sister.”
Select it to continue toward the True Ending.
Monkey Happy Ending
This ending is only available after completing the Normal Ending.
Start a new game and select the Monkey option on the panopticon screen.
The “He’s not a bad guy” choice will now be available.
Select it to get the Monkey alternate ending.
CG Unlocked

If you tried doing the Monkey route before getting the Normal Ending and couldn’t get this ending, that’s expected. The alternate Monkey ending requires the Normal Ending first.
Caterpillar Bad Ending
For the Caterpillar Bad Ending, go to the rooftop and talk to Igusa.
When you get the choice, select anything other than:
“You can do it. You can fly.”
This will lead to the Caterpillar Bad Ending.
There are no additional CGs specifically tied to this ending.
CGs 21–25
You can get CGs 21 to 25 randomly while progressing through the route segments.
You can get them after completing the Normal Ending, and they may appear after finishing different route segments.
If you are still missing some of these CGs, there is an easier way to farm them.
Save your game after the cutscene with Sahana, when she speaks from the center of the screen with a black background and no character sprite.
From there, keep reloading the save until you get the CG you are missing.
Once all 25 CGs are unlocked, the image gallery will be complete.
